Each player must complete the LITTLE ROCK JUNIORS ONLINE REGISTRATION. Bring two completed copies of this Medical Release form with you to tryouts/evaluations. Bring two completed copies of this Player Information form with you to tryouts/evaluations. If you were not a member of Little Rock Juniors last season (2010 - 2011), you must present proof of your date of birth.  Acceptable proof can be a birth certificate or a player’s driver’s license. We will NOT retain these documents (or copies thereof); they will only be used as proof of the player’s date of birth. Returning USAV Player Registration New USAV Player Registration View Information on Tryouts and Program Cost After Completing your USAV registration you MUST PRINT A COPY OF YOUR USAV MEMBERSHIP Card.  YOU will be required to present a copy of your membership card at tryout check-in as proof of your USAV registration.  Players that do not provide proof of USAV memberbship will not be allowed to tryout.  THIS COPY WILL BE RETAINED FOR OUR RECORDS.
